Utah’s Invisible Condition Alert Program

Utah's Invisible Condition Alert Program, established under S.B. 148, is a voluntary program for individuals with a qualifying physical or mental condition. Participants may have a symbol added to their Utah driver license or identification card.

The symbol notifies first responders that the cardholder has an invisible condition that may affect their ability to communicate.

How to Apply

A qualifying Utah health care professional, as defined in Utah Code 53-3-207, must sign the Invisible Condition Request form. The completed form may be submitted to the Utah Driver License Division (DLD) by mail, fax, email, or in person at a DLD office. If you believe you have a qualifying physical or mental health condition and wish to apply for this program, you must discuss this with your healthcare provider and your provider must attest to your condition by completing the provider portion of the application form.
